Prescription Management

Synod provides a multi-tier medication management system that allows our elderly and disabled clients to maintain and improve their health. The service can include:

  • Confer and schedule with physicians
  • Organize and set up medication boxes
  • Eyes on delivery and administration of medication
  • Inventory and re-ordering
  • Education and self-medication training

Despite the fact that recovery from many illnesses is possible only through the consistent and proper adherence to daily medication regimens and treatment protocol, an estimated one-third to one-half of all patients in the U.S. do not take their medications as prescribed. Poor adherence and medication errors often result in the worsening of disease, excess hospitalizations, and can endanger the lives and well being of seniors, mental health consumers and others who have difficulty tracking which medications have been taken and which have not. (1) 

The problem does not end with a patient’s willingness to participate in treatment, since many individuals, suffering from mental health and other cognitive related disorders have difficulty “tracking” which medications have been taken and which have not.

Synod’s medication and prescription management service addresses this safety and quality of life issue by providing graduated support ranging from medication delivery and administration to telephone prompts and reminders – ensuring that medications are delivered as prescribed.
